The ACCESS Program
of Project AWARE provides intake, assessment and case management
services to those affected by and/or living with HIV or AIDS. The
case management component of our department is staffed by professionals
who assist HIV-positive people navigate the many complex medical and social
service systems to coordinate care and plan for service delivery.
The ACCESS Program assesses HIV-related needs
and concerns along with assisting with access to necessary services.
Case
Management works one-on-one to help individuals navigate through the
social service maze. Staff assists consumers in maximizing
available service options providing practical and emotional support.
Together with other programs
at SSTAR and in the community; support, counseling, drug treatment
and medical services can be obtained. Your first visit
with a Project AWARE staff member will be to conduct an intake and
assessment of your needs regarding the delivery of any HIV-services
you may need. This intake will last approximately 45 to 60
minutes depending on the number of issues to be discussed, services to
identify and/or offer, and the number of questions you may have about
your care.
In order to make this
first visit a comfortable experience, we have prepared a checklist of documents to bring with you. Your intake worker
or case manager will need most of this information in order to create
a complete client file and identify the best services to meet your
needs. In addition, external services that you might need, such as
social security, medical insurance or help with paying the cost of
medications and drug co-payments also require verification of your
residency, income and HIV status.
