In addition to the services offered by the HIV
ACCESS Program, Project AWARE also provides on-going and
transitional case management for its registered clients through its HIV
Client Services Unit. These direct care services are for people
living with HIV disease and/or AIDS as well as their loved ones.

level of consumer need.
These services are social in nature and are not intended to address
medical issues. As needed, your Case Manager will assist you
with a referral to appropriate medical services. |

Project AWARE is staffed by professionals
who assist HIV-positive people navigate the often complex medical and
social service systems to coordinate care and to plan for service
delivery. Our program is funded by
through the
Massachusetts Department of Public Health, The
City of Fall River Community Development Agency,
Justice
Resource Institute and private and corporate donations. All
services are confidential and can be provided in English, Spanish and
Portuguese. |
