SSTAR-Care Community Partners is a community-based MassHealth program that provides care coordination and management to eligible MassHealth members

SSTAR Care Community Partners (CP) is a community-based MassHealth program designed to provide care coordination and management to MassHealth members. Eligibility is determined by MassHealth based on need and enrollment in an Accountable Care Organization (ACO) and Managed Care Organization (MCO). SSTAR-Care CP is a partnership among HealthFirst Community Health Center, Greater New Bedford Community Health Center, Fellowship Health Resources and SSTAR and we serve members from Brockton-Quincy to Cape Cod. Our primary purpose is to identify needs, reduce barriers, and improve health outcomes by linking members to resources in their communities. Members are matched with an interprofessional team, including a Care Coordinator, Registered Nurse, and Clinical Social Worker, to address immediate needs and develop short- and long-term goals.

CP takes a patient-centered approach by working closely with the member and collaborating efforts with formal and informal supports to achieve comprehensive care. CP services include support with accessing medical and mental health providers, substance use services, transportation, affordable housing, income, education, and many other resources. Our Care Coordinators are mobile, meeting with members in the comfort of their community. We provide services in English, Spanish, Portuguese, Cape Verdean Creole, Bengali, and American Sign Language.

In order to enroll in the program, please talk with your PCP/Health or behavioral healthcare provider about a referral. In some instances, we can make the referral directly, just contact our main number. We do have a MassHealth certified Navigator on staff who can assist with insurance coverage issues.
SSTAR-Care CP serves a large geographic region that includes Brockton, Quincy, and towns to the south including Cape Cod. We serve towns to the west as far as Seekonk.
Mass Health determines eligibility based on your medical and/or behavioral health needs. Members may have chronic medical issues, mental health issues and/or substance use issues. Member eligibility requires enrollment in an ACO such as Southcoast, Steward, Signature or Boston Accountable Care Organization (BACO). If you belong to a different ACO, we can determine which Community Partner program is appropriate for your coverage. The program is not open to people under the age of 18 or people who receive Medicare/Medicaid for health coverage.
We provide care coordination for our members (including nursing). These services include help with transitioning into or out of a hospitalization, help with housing applications, public assistance, transportation, food and clothing, communications and other services that support your well-being.
